Additionally, subsequent consumer reports may be requested on renewal as an update on your insurance application. Upon your written request, you will be informed whether or not a consumer report was requested, and if so, the name and address of the consumer-reporting agency to whom the request was made. You also have the right to inspect and receive a copy of this report by contacting the consumer-reporting agency. Under the New York Fair Credit Reporting Act we are required to obtain your authorization for requesting and investigative consumer report. Please indicate this authorization below.
